What Could a Dataset for Licenses, Permits or Credentials Look Like?

Many of our state agencies have a regulatory mission, and ensure compliance with regulations through the issuance of licenses, permits and/or credentials.  While each type of license, permit and credential is different, there are similarities for what we might share regarding the individuals or companies regulated.  Doing so provides fundamental information on what we do.  Below is an example of what such a dataset could look like.

Data Element Description
ID Provide the unique ID for the license, permit or credential.
Name Provide the name of the individual or organization the license, permit or credential was issued to.
Type Provide the type of license, permit or credential that was issued. This is necessary for any dataset that may include multiple types having different conditions and/or requirements.
Description Provide a summary of the license, permit or credential's requirements (i.e., capacity or operational limitations). This would not be necessary if the requirements are the same for all licenses, permits or credentials included in the dataset or are the same within each "type" and can be explained in the dataset's metadata.
Status Current status of the license, permit or credential (i.e., active, active - corrective action, inactive, revoked, expired, etc.)
Status Reason Explanation for the current status if necessary.
Issue Date Date the license, permit or credential was issued by your agency.
Effective Date Date the license, permit or credential became effective.
Expiration Date Date the license, permit or credential expires.
Document If available, provide URL to license, permit or credential.
County FIPS Where a specific location cannot be shared for confidentiality purposes, or to provide a means to filter or group data, consider providing the federal information processing code of the county for which the license, permit or credential was issued. A federal information processing series five-digit code for the identification of counties and county equivalents of the United States and its insular and associated areas, for the purpose of information interchange among data processing systems. (INCITS 31-2009).
County Where a specific location cannot be shared for confidentiality purposes, or to provide a means to filter or group data, consider providing the name of the county for which the license, permit or credential was issued.
Primary Address Provide the primary address of the individual or organization the license, permit or credential was issued to. Primary address should include: number, predirectional (if applicable. i.e., N, S, E, W, NE, NW, SE, SW), street name, street suffix (i.e. street, avenue, etc.) and postdirectional (if applicable). This should only be provided for place of business or place of operation, not personal residence.
Secondary Address Provide the secondary address, if applicable, of the individual or organization the license, permit or credential was issued to. Secondary address should include: unit designator (i.e. APT - apartment, BLG - building, FL - floor, STE - suite, UNIT, RM - room, DEPT - department, etc.), unit number. As with primary address, this should only be provided for place of business or place of operation, not personal residence.
Post Office Box Provide the post office box information (i.e., post office box designator (e.g., PO Box), post office box number) of the individual or organization the license, permit or credential was issued to.
City Name Provide the city of the individual or organization the license, permit or credential was issued to.
State Provide the state of the individual or organization the license, permit or credential was issued to.
Zip Code Provide the zip code for the individual or organization the license, permit or credential was issued to.
Latitude Latitude in decimal degrees, if available, associated with the business address or place of operation of the individual or organization the license, permit or credential was issued to.
Longitude Longitude in decimal degrees, if available, associated with the business address or place of operation of the individual or organization the license, permit or credential was issued to.
